OPT is a fully independent and physical therapist owned, Limited Liability Corporation (LLC), committed to delivering high quality physical therapy and wellness services to our clients. Quality is provided by working closely with referring physicians/medical providers, patients and payors (insurance company, workmans comp, medicare, etc.).
OPT was established in August 1987. Over these 30+ years, OPT has served thousands of its clients in and around Jefferson, and prides itself on the consistently superior feedback about our services, from a broad spectrum of our clients.
We participate in a number of plans including Aetna, United Healthcare, Horizon BC/BS etc. and several healthcare networks. We are a Medicare provider. Please provide specifics when you call for a physical therapy appointment.
Our office will verify coverage with your insurance carrier, and also expect you will contact your carrier prior to your first visit, to check on any limitations your policy may have regarding physical therapy. You must check with your insurance, because, many plans require a physician referral, and it takes time and effort to obtain authorizations. You may need a referral and/or authorization depending on your insurance plan before our therapist can see you.
Our office accepts cash or credit card as payment of copays, deductibles and uninsured amounts.

Please obtain a letter of authorization from the payor. The authorization letter must include the name of payor or insurance company, their address, the name and phone number of the case manager, claim number assigned to the case. You may bring along this letter at the time of your first visit, or our office may request you to fax them a copy in advance of your visit.
